As a part of my volunteer experience with Empowerment Word Church (EWC), I understand that my name, image (in photograph and/or video), or statements (collectively “contribution”) may become a part of a promotional video or product. I give EWC and its publishers and partners the right to use my “contribution” in all forms and media for use in advertising and promotion. I waive my right to approve of or inspect the use of my name, image, and/or statement in the product and acknowledge that I have no ownership rights in the product. I release EWC and its assigns, licensees, and succors from any claims that may arise regarding the use of the “contribution” including any claims of defamation, invasion of privacy, or infringement of moral rights, rights of publicity, copyright or right to compensation. I have read and understood this agreement.
As a member of E-Kids/ E-Youth, I understand that I may come into contact with personal information about kids, teens and their families that could be sensitive and should be kept confidential. I agree to keep all information about children's/youth’s health, behavior, disabilities, and related matters confidential and only disclose it to EWC Staff who need to know. I agree that I will uphold this confidentiality statement whether or not I continue to be a member of E-Kids or E-Youth, in order to protect the families, children and youth.
